Healthy Weight Collaborative Grant

 

The New York State Public Health Association (NYSPHA) is pleased to have been awarded a grant for Phase II of the Healthy Weight Collaborative, a key initiative of the Collaborate for Healthy Weight project.

The Healthy Weight Collaborative is a project of the National Initiative for Children's Healthcare Quality and funded by the Health Resources and Services Administration (HRSA). NYSPHA's project focuses on the City of Troy in Rensselaer County.

The Healthy Weight Collaborative is a national initiative to bring together a team comprised of primary care, public health and the community to reverse the obesity epidemic in communities across the country. Using quality improvement methods, these three groups are working together in Rensselaer County to make an impact on the pediatric rates of obesity.

This project was funded in March 2012 for a period of one year. The project team is coordinated by the New York State Public Health Association and the team includes representatives from the following programs:

·         Seton Health Pediatrics

•·         Seton Health Pediatric Obesity Prevention Program

•·         Capital District Community Gardens

•·         New York State Healthy Eating and Physical Activity Alliance

•·         New York State Healthy Heart Program

The New York State Public Health Association team is committed to designing and implementing strategies to improve healthy weight status and health equity in Rensselaer County. By February 28, 2013, we are committed as a team to use evidence-based interventions to promote healthy weight among our target population, Seton Health Pediatrics in Troy, New York, specifically patient’s ages 2 to 18 years old.  Our improvement will be evidenced by achieving the following goals:

•·         By February 28, 2013, 85% of children ages 2 – 18 seen for well care in participating ambulatory care sites have a completed weight assessment

•·         By February 28, 2013, 85% of children ages 2 - 18 seen for well care in participating ambulatory care sites are offered a healthy weight plan

Based on similar goals and objectives, NYSPHA’s Healthy Weight Collaborative team has partnered with Seton Health’s Pediatric Obesity Prevention Program to work towards achieving the goals of this project.
